Person Centred Approach

Supported Lives is an ethical organisation, that aims to puts quality before profit. We are dedicated to working in creative and positive ways with people who have disabilities. We will put together individualised, person-centred services to assist people in their own homes, their local communities and/or to access valued occupation and daytime activities. The aim of the service is to work within the spirit of the government white paper, Valuing People, providing individually planned services of the highest quality given the funding available.


Supported Lives is part of the Potens Group, who deliver health & social care support services for children and adults with disabilities and complex needs nationwide.

Services provided by Supported Lives

Home and Community based support

Supported Lives offers support both in and outside of their home, putting together individualised packages of support to best meet each person’s needs.

This could include encouragement or support to help the individual get ready for their day, manage their medication, assist with their personal care or nutritional needs, support to enable them to attend or take part in social / educational activities or support to play a fuller part in their family life and community.

This could also be a much more intensive package of support for people who have high-level support needs, or needing more than 1 person to safely support them in achieving their outcomes.

Supported Living

We support people in their own homes, ranging a couple of hours a week to manage their home, right up to supporting people 24 hours a day, 365 days of the year, with an assessed and individually designed package of support, aimed at promote independence and inclusion in their local community.

Alternative day services

Supported Lives offers an alternative to traditional day service in which each individual member has a programme of activity that best meets their needs and wants. We will make the service offered as individual as possible and one that would help someone take part in activities which would normally be closed to them because of their support needs.
